Non NHS Examinations/ Medicals / Certificates / Forms etc

Medical Examinations for special purposes, e.g. HGV Driver, can be arranged by appointment. As this is not part of your Doctors NHS duties a fee will be charged for this work. A list of the Surgery Charges for this and other work is displayed at reception.

Medical Certificates

For absence from work through sickness, you will be able to ‘self certificate’ for the first six days. The sick certificate (Form SC1) should be provided by your employer or is available from the Post Office. If a Private Certificate is requested, to cover this absence, a fee will be charged for issuing this. After the first week NHS certificates can be obtained from the Doctor. If you are under the care of a Hospital Doctor you may ask them to issue your certificate for an in-patient stay, or at an out-patient appointment for an on-going condition.
